FMGs should do their rotations, USCE which are hospital-based and not clinic-based.
By hospital-based, we mean more emphasis is laid on the hospital and the work there in. Even if the rotation is for a day or two in the hospital, heavy emphasis and training is done by attending physician for the hospital exposure and patient care.<\/p>\n\n\n\n

Most of the rotations currently being done by FMGs are clinic-based, meaning most of the days and hours they are in clinic seeing the patients. When the FMGs are selected for residency, 95% of their work is in the hospital and in-patient care. Hence, doing rotations that are hospital and in-patient based makes more sense.<\/p>\n\n\n\n

Rotations can be observerships or electives. ELECTIVES are usually reserved for people in their fourth or final year of medical school. OBSERVERSHIPS are reserved for people in their internships or after graduation. The other important term is \u201cHANDS ON\u201drotations. Hands-on rotations are those rotations where you interact with the patient, talk to the patient, do the History & Physical, and do the physical examination. Also, form your independent assessment and plan for the patient, and discuss your plan with the attending physician.<\/p>\n\n\n\n

Hands-on does not mean that you have access to electronic medical records, or that you are writing EMR notes or writing orders. Almost \ud83d\udcaf % Hospitals in US don\u2019t allow it!!
Hands-on means interaction with the patient and direct interaction with the attending physician in the patient\u2019s care.
Student should seek such clinical rotations that will get them good letters of recommendation.<\/p>\n\n\n\n

What do we mean by a good letters of recommendation?<\/h2>\n\n\n\n

These are LORs that are given by important faculty people like chairman of department of medicine, vice-chairman, section chiefs, or directors of ICUs or critical care units. These faculty designations are crucial and are weighted heavily.<\/p>\n\n\n\n
